Estella Byrd Whitman Community Health Center offers Family Medicine services devoted to comprehensive primary health care for the whole family beginning at the age one, treating a large variety of different health conditions including management of chronic diseases, illnesses, and childhood disorders.

Point-of-Care-Testing Basic Labs

Estella Byrd Whitman Community Health Center offers Point-of-care labs (POC) at patient’s appointments as determined by their Provider. These lab tests are designed to screen disease. Point-of-care testing involves performing diagnostic tests that produces rapid and reliable results helping to manage or identify chronic disease and acute infections.

Specialty Care Referrals

Estella Byrd Whitman Community Health Center values its patients. Our Patient Experience Liaison meets each patient on their first appointment and will continue to guide patients through the healthcare system with their in-patient and post-hospital experience and needs. They ensure patients understanding of their options, rights and responsibilities, while providing support and advocacy to enhance their overall experience.

Direct Patient Care Coordination

Estella Byrd Whitman Community Health Center treats the whole person, which includes assessing our patient’s needs beyond medical conditions. Our Patient Care Coordinator contributes to the Provider’s treatment plan, advise patients about resources, services, and programs. Continued follow-up communications with the patient to ensure the services and resources are provided and that the patient has completed referral process and their needs have been met.

Chronic Care Management

Our Clinical Team provides a comprehensive approach to supporting patients with long-term health conditions by coordinating their care across multiple Providers and settings. It aims to optimize patient outcomes by addressing both medical and non-medical conditions. Including medication management, lifestyle adjustments, and emotional support. This proactive and coordinated approach helps patients better understand and manage their conditions.

Physical and Wellness Exams

Annual physical and wellness exams are a part of a patient’s comprehensive primary care. These exams assess overall health, including vital signs, medical history, and screenings for various conditions, promoting preventive care and healthy lifestyle choices.

Health Education

Patient-Centered Care includes teaching patients about making healthy choices and preventing illness through knowledge and skill-building. Estella Byrd Whitman Community Health Center wants its patients to partner with their Provider to be empowered to take control of their well-being by providing information on topics as nutrition, exercise, and disease prevention.

Drug Assistance Program

Estella Byrd Whitman Community Health Center offers The Drug Assistance Program (DAP) that provides affordable access to essential medications for individuals facing financial barriers. We cover a wide range of medications, offer confidential support, and assist with the application process.
